2011 City/Village Income Tax Return

As a result, the Regional Income Tax Agency (R.I.T.A.) is discontinuing the automatic mailing of tax forms. If you choose to file a paper return, tax forms can be obtained at your city/village hall, public libraries, online at www.ritaohio.com or by calling 800.860.7482. The following will help you determine which R.I.T.A. form to use.
Use Form 37 if: Use Form 37B if:
You have income other than W2 income You have W2 income only
You moved last year You did not move last year
You live in a REDUCED Credit municipality You live in a FULL Credit municipality
