Individuals must be current clients of the Cuyahoga County Department of Employment and Family Services and receive food assistance to qualify.

Instructions for Replacement of Food Destroyed Due to Fall Wind Storms
- Complete the JFS-07222 - Statement Requesting Replacement of Food Stamp Benefits. You may complete the JFS-07222 in person, via telephone or by voicemail.

- The JFS-07222 may be completed and signed at the front desk at each Neighborhood Family Service Center
- Telephone
- You may call your worker and have the caseworker complete the application on your behalf
- Voicemail
- You may leave a message on voicemail requesting the replacement of Food Stamp benefits. You will receive a return phone call from your worker.
- You must provide verification that a power outage of 4 hours or more occurred. If you do not have verification, your worker will assist you in obtaining it.
- The deadline for requesting the replacement of Food Stamp benefits is 10 days from the date of the loss.
- Once authorized, replacement Food Stamp benefits will be available via the Ohio Direction Card to the client no later than 24 hours after the start of the next business day.
