Mail the form or place it in the Community Services drop box located outside the main entrance of Beachwood Community Center.
The Cuyahoga County Board of Health will provide flu shots to the Beachwood Community Center. Shots are free if you have Medicare (not a Medicare HMO). For Aetna, Anthem, Kaiser, SummaCare, United Healthcare and Humana, the cost will be $30.00. An insurance form will be available for you to submit for possible reimbursement. Flu shots may also be available at your physician’s office. Call the Cuyahoga Board of Health with any questions at 216-201-2001.
The 2010-2011 flu vaccine will protect against 2009 H1N1, and two other influenza viruses (an H3N2 virus and an influenza B virus). You still need the 2010-2011 if you received a H1N1 vaccine last year.
The Cuyahoga Board of Health has sole control over the number of vaccinations provided to the City of Beachwood. The number of vaccinations provided by the County is limited and may be insufficient to vaccinate every Beachwood resident who requests a flu shot. Because vaccines are limited, reservations will be accepted only until the County allocation for Beachwood is met. Please keep in mind that because there may be more individuals requesting vaccinations than there are allocated vaccines, responding before the October 8 deadline does not guarantee you a flu shot.
Shots administed continuously from 9:00 a.m. – 12:00 Noon
No appointments needed, but registration form must be filled out and turned in by October 8
